Join our dedicated team in San Jose, CA, as a Psychiatric Mental Health Nurse Practitioner in a hybrid remote role, where compassion meets comprehensive care! In collaboration with the Medical Director, PACE Physicians, and the Interdisciplinary Team (IDT), you will play a key role in delivering vital psychiatric services. Your expertise will help seniors live comfortably in their homes and communities while you provide valuable assessments, planning, primary care, and ongoing education.
Key Responsibilities:
- Deliver psychiatric consultations and evaluations, along with therapeutic counseling, in partnership with PACE Physicians and community-based professionals.
- Conduct comprehensive psychiatric assessments—both in-person and via telehealth—and craft individualized care plans with the IDT.
- Assist in discharge planning efforts for participants transitioning from acute or long-term care facilities.
- Collaborate with physicians to refer participants to psychiatrists when necessary.
- Visit and oversee patient care in nursing homes, ensuring each participant’s needs are met appropriately.
- Act as a consultant for the IDT, sharing updates on participant conditions and facilitating seamless care coordination.
Qualifications:
- Hold a Master’s degree in nursing with a Psychiatric/Mental Health focus from an accredited institution.
- Certification as a Psychiatric Mental Health Nurse Practitioner is essential.
- Possess an unrestricted active Psychiatric Mental Health Nurse Practitioner State License.
- Be equipped with State CDS Certification, a DEA License, and an NPI license.
- A valid State Driver’s License with a clean DMV record and proof of current vehicle insurance is required.
- A minimum of two years’ experience in Psychiatric Mental Health care, particularly with elderly or frail populations.
Essential Credentials:
- Certification as a Psychiatric Mental Health Nurse Practitioner
- Active California Nursing License
- Current DEA registration

The salary for this position, combined with our total rewards package, reflects our commitment to competitive compensation, starting at $160,000.00 and going up to $185,000.00 + bonuses and equity. Actual pay will be tailored to experience and qualifications.
