In the role of Health Home Field Care Coordinator, you will serve as the key case manager for a designated group of high-cost, high-risk Medicare-Medicaid enrollees. Your primary focus will be on providing intensive care coordination services that enhance healthcare outcomes and drive cost savings. You will seamlessly integrate care across various delivery systems, addressing service gaps while ensuring a coordinated approach among all service providers, including medical, behavioral health, long-term services and supports, and other social services. Through engaging with patients and applying the principles of activation and empowerment, you will help set health action goals and boost self-management skills to achieve optimal physical and cognitive well-being.

Your Key Responsibilities Will Include:
- Engaging with members, families, and caregivers through face-to-face interactions to conduct comprehensive health needs assessments that cover medical, behavioral, functional, cultural, and socioeconomic factors.
- Creating and executing personalized Health Action Plans that address chronic health conditions, promote wellness, manage medications, and ensure member safety, all aligned with evidence-based guidelines.
- Collaborating with internal teams, providers, and community resources to implement care strategies effectively.
- Providing education, health promotion, and coaching that empowers members to manage their own care and embrace lifestyle changes for better health.
- Offering individualized and family support tailored to each member’s unique situation.
- Aiding in proactive discharge planning and coordinating care transitions following emergency room visits or admissions to inpatient or skilled nursing facilities.
- Advocating for members and families to ensure their needs and preferences are recognized and supported within the healthcare team.
- Researching and connecting members with community resources to assist with specific care or socioeconomic challenges, while following up to confirm service delivery.
- Proactively engaging members in managing their health care, offering ongoing education and coaching as outlined in their care plans.

To Be Successful in This Role, You Should Possess:
- A Bachelor’s Degree in Social Work, Psychology, Social Services, Human Services, or Behavioral Sciences or equivalent qualifications such as a Certified Medical Assistant with an associate degree, Licensed Practical Nurse, or Licenses in mental health counseling, or a Master’s Degree in related fields.
- The ability to obtain state certification as a Health Home Care Coordinator within six weeks of your hire date.
- At least 3 years of relevant clinical experience.
- Experience working with MS Office tools including Word, Excel, Outlook, and Teams for a minimum of 1 year.
- Reliable transportation for travel up to 75% within King County to connect with members and providers.
- Residence within Seattle, WA – King County or proximity to commuting distance.
Preferred Qualifications Include:
- CCM certification.
- 1+ years of experience in community case management focused on coordinating care for individuals with complex needs.
- A background in Managed Care.
- Experience participating in team-based care settings.
- Bilingual capabilities.
Physical Requirements for This Role:
- Ability to transition frequently between office and field settings throughout the day.
- Capacity to navigate various locations to visit members, providers, and collaborators.
- Ability to transport necessary equipment such as laptops to and from field locations.
- Capability of remaining stationary for extended periods to handle computer or tablet tasks.

The hourly wage for this position ranges from $23.70 to $46.35 for full-time employment. Compensation is determined by several factors, including local labor markets, education, experience, and certifications. UnitedHealth Group adheres to all applicable minimum wage laws.
