    In this role, the RN Clinical Care Coordinator will function as the primary care manager for a dedicated panel of members facing complex medical and behavioral challenges. Your focus will be on coordinating care activities that address our members’ medical, behavioral, and socioeconomic needs—ensuring optimal service utilization and elevating the quality of care provided.

    This position is home-based with essential fieldwork; you’ll be engaging in community interactions approximately 50% to 75% of the time within your designated coverage area.

    Candidates should reside in Lima, OH or Allen County and be open to commuting to nearby counties.

    If you’re located in Allen County or surrounding areas, this role offers you the flexibility to work remotely while tackling significant challenges.

    Your Key Responsibilities:

    • Engage with members both in-person and via phone to conduct comprehensive needs assessments, covering medical, behavioral, functional, cultural, and socioeconomic dimensions.
    • Craft and execute personalized care plans addressing diverse needs such as chronic condition management, health promotion, lifestyle changes based on social determinants of health, medication oversight, and member safety, in line with best practices.
    • Collaborate effectively with internal teams, providers, and community partners to implement care plans.
    • Provide education and guidance to empower members in self-managing their health and making positive lifestyle adjustments.
    • Facilitate proactive discharge planning and coordinate care transitions following emergency room visits or admissions to inpatient facilities or skilled nursing facilities (SNFs).
    • Advocate on behalf of members and their families to ensure their needs and preferences are fully communicated and upheld within the health care team.

    What You’ll Need to Bring:

    • A current, unrestricted RN license in Ohio.
    • A minimum of 2 years of clinical experience as a Registered Nurse.
    • Proficiency in MS Office (Word, Excel, Outlook) for at least one year.
    • A reliable mode of transportation with the ability to travel up to 75% within Allen County and surrounding areas to meet with members and providers.
    • Residency in Lima, OH or Allen County as well as Putnam, Hancock, or Hardin counties.

    Preferred Qualifications:

    • A Bachelor of Science in Nursing (BSN) or higher degree in a clinical field.
    • A certification in case management (CCM).
    • At least one year of experience in community case management, specifically coordinating care for those with complex health needs.
    • Experience working collaboratively in a team-oriented care setting.
    • A background in Managed Care.

    The hourly compensation for this role ranges from $28.61 to $56.06, depending on full-time status. Pay is determined by various factors such as local labor markets, education, experience, and certifications. UnitedHealth Group follows all applicable minimum wage regulations.
