Description:
As part of our dynamic team, you will play a crucial role in the processing and analysis of physician/provider documentation within designated Emergency Department (ED) and Outpatient Observation health records, whether they be electronic, paper, or a hybrid of both. Your expertise will be key in determining principal and secondary diagnoses, as well as principal and secondary procedures. You will also access charge work queues and systems to assign Emergency Room and Observation charges when performed by Health Information Management (HIM). Additionally, you may need to calculate Observation hours if required.
You will leverage encoder software applications and all relevant online tools and references to accurately assign Internal Classification of Diseases, Clinical Modification diagnosis, procedure codes, and Current Procedural Terminology / Healthcare Common Procedure Coding System (HCPCS) procedure codes along with necessary modifiers.
Your work will adhere to coding guidelines established by various reputable organizations, including:
– The Centers for Medicare/Medicaid Services (CMS) ICD-CM and ICD-PCS Official Coding Guidelines
– The American Hospital Association (AHA) Coding Clinic
– The American Medical Association (AMA) for CPT codes and CPT Assistant
– The American Health Information Management Association (AHIMA) Standards of Ethical Coding
– Revenue Excellence/Health Ministry (HM) coding procedures and guidelines
Essential Functions
You will embody and promote the Trinity Health Mission, Vision, and Values in all your actions and decisions. Your responsibilities include navigating the patient health record and other systems for determining diagnoses, reasons for visits, procedures, and the appropriate modifiers for coding and APC assignment.
Your expertise in the Emergency Department and Outpatient Observation coding will involve utilizing encoder software and online resources for assigning ICD, CPT, and HCPCS codes and modifiers. You will manage charge work queues to assign ER and Observation charges and hours based on the medical record documentation, engaging with HIM at our Health Ministry.
You will also be knowledgeable about compliant coding query practices and will collaborate effectively with physicians, Clinical Documentation Specialists (CDS), and other healthcare providers when further information is required. Engaging with clinical staff regarding documentation issues and communicating with case management regarding Outpatient Observation documentation will be essential.
Partnering with HIM and Patient Business Services (PBS) teams, you will help resolve billing and claims issues that impact reimbursement. You will remain vigilant about documentation ethics concerns, reporting any issues to leadership when necessary. You will utilize EMR communication tools to track missing documentation and Outpatient queries to ensure timely coding.
Staying updated on Outpatient coding and reimbursement guidelines, you will consistently meet or exceed the coding quality and productivity standards established by Revenue Excellence/HM, while also maintaining your Continuing Education Units as required by your coding credentials. You will fulfill other duties as directed by leadership and maintain compliance with applicable regulations and codes of conduct.
Minimum Qualifications
To excel in this role, you must have completed an AHIMA or AAPC-approved coding program or hold an Associate’s degree in Health Information Technology or a related field, or possess an equivalent combination of education and experience. A Bachelor’s degree in Health Information Management (HIM) or a related healthcare field is preferred.
You will need to hold one of the following certifications: Certified Coding Associate (CCA), Certified Procedural Coder (CPC), Certified Outpatient Coder (COC), Registered Health Information Technician (RHIT), Registered Health Information Administrator (RHIA), or Certified Coding Specialist (CCS). A minimum of two years of recent acute care coding experience in emergency department and observation settings is required, with current proficiency in encoding/grouping software preferred.
Strong oral and written communication skills are essential for effective interaction with diverse groups. You should be capable of working independently and exercising sound judgment, alongside your ability to research, analyze, and assimilate information based on technical and experience-driven knowledge. Excellent organizational and customer service skills are crucial, as is the capacity to adapt to changing priorities.

Physical and Mental Requirements and Working Conditions

Your role may require travel to different Trinity Health locations as needed, and if telecommuting, adherence to the organization’s remote work policy is expected.
Hourly Pay Range: $24.05 – $36.08
