Your primary responsibilities will include conducting comprehensive assessments that cover a range of factors, including psycho-social, physical, medical, and environmental needs, to create tailored care plans. You will act as the main point of contact for clients and their families, facilitating effective communication and ensuring that all services are delivered efficiently during transitions, such as when moving to home care or utilizing community resources.
In this role, you will implement innovative strategies to enhance health outcomes and quality of life for members and their families. You will review and adjust care plans regularly, identifying any gaps or trends in care while advocating for the needs of your clients. Continuous monitoring and evaluation of the care plan’s effectiveness will be crucial, and you will be tasked with collecting and analyzing data to drive improvements in service delivery.
You will collaborate proactively with an interdisciplinary care team, which may include healthcare providers, caregivers, and other stakeholders, to address each member’s unique needs—whether they pertain to medical, behavioral, social, or community-based services. Your ability to provide clear guidance and education on care coordination will empower both families and healthcare providers to implement best practices in member-focused care.
To thrive in this position, we seek candidates with 3-5 years of experience in Social Work, Nursing, or a related healthcare field, along with a solid background in utilization management, quality assurance, and community health. Familiarity with cost-benefit analysis and decision-making processes is essential, as you will be responsible for navigating complex situations that require innovative solutions. Excellent communication skills are a must, allowing you to engage effectively with clinicians, community resources, and clients alike.
