As a Telephonic Triage Nurse Case Manager, you will have the opportunity to work from the comfort of your home while remaining an integral part of a nationwide support team. Utilizing your nursing expertise and case management skills, you will assist federal employees dealing with occupational injuries and illnesses. Your role will be pivotal in aiding their recovery through effective communication, timely diagnosis facilitation, and comprehensive treatment coordination. You will formulate personalized case management plans that evolve through the recovery journey, all while tracking progress in our database. Our focus on integrated medical disability case management ensures we can help prevent or mitigate disabilities, offering essential medical insights and serving as the key connector among all stakeholders in the recovery process.
Key Responsibilities
- Accept and manage referrals from DOL within specified service periods.
- Ensure timely communication and information sharing with injured workers, healthcare providers, and relevant agencies.
- Diligently document all contacts and communications in the designated case management systems.
- Conduct comprehensive assessments of each injured worker, considering both clinical and psychosocial factors.
- Work collaboratively with injured workers, providers, and employers to develop and execute effective return-to-work plans.
- Secure necessary medical documentation to support return-to-work initiatives.
- Understand discharge criteria for injured workers transitioning out of Telephonic Triage services.
- Identify cases requiring additional case management services such as DOL Field Nurse assignment.
- Foster positive relationships with all parties involved, enhancing communication and coordination.
- Comprehend the quality management program and your essential role within it.
- Be an active contributor to the team, supporting colleagues and meeting organizational goals.

- Exhibit proficiency in Microsoft applications and electronic technology.
Qualifications
- Bachelor of Science in Nursing.
- Active RN License from Puerto Rico is required; must reside in the continental US.
- Compact License is necessary.
- Preferred national certification in case management or a related area (COHN, CCM, CIRS/CRRN, CDMS, or ANCC/ANA Nurse Case Manager Board Certified), or to be obtained within 12 months of hire.
- Five years of relevant clinical experience, with:
- At least 2 years in adult medical/surgical nursing.
- At least 2 years in workers’ compensation case management.
- Preferred: Familiarity with the Federal Employee Compensation Act (FECA) and its associated issues.
- Demonstrated ability to work independently and collaboratively.
- Detail-oriented with strong organizational and judgment skills.
- Experience with Microsoft Windows and basic computer operations.
- Reliable high-speed cable or fiber optic internet service is a must.
- Exemplary communication skills, able to engage effectively via phone, email, text, and written formats.
- Willingness to undergo a credit and background check prior to employment.
