Our Home and Community Care (HCC) program is a comprehensive, integrated approach to healthcare delivery. As an advanced practice clinician (APC) in our National On Call team, you will deliver telephonic care and guidance to members and facility staff during evenings, overnight shifts, weekends, and holidays. Your medical services will directly impact a designated group of enrollees, providing them with essential care remotely, primarily through phone consultations. This position offers autonomy and the ability to achieve a healthy work-life balance while transforming care delivery with personalized solutions. Scheduling Details:

• Enjoy the flexibility of a full-time work-from-home position with varied shift coverage required during weeknights, weekends, and holidays.
• Our commitment to our members includes availability during:
  • Weeknights from 5 PM to 8 AM local time
  • Continuous weekend coverage from Friday at 5 PM through Monday at 8 AM
  • Holiday coverage starts at 5 PM on the last business day before the holiday and continues until 8 AM when business resumes.
• Weekly shifts primarily fall between 5 PM and 8 AM.
• Weekend shifts are scheduled from 12 AM to 12 PM.
• Holiday scheduling is planned annually for your convenience.

Ideal Experience:
We prefer candidates with recent high-acuity geriatric care experience, including but not limited to skilled nursing facilities (SNF), long-term acute care hospitals (LTACH), hospitalist, ICU, or emergency care settings.

Call Handling:
You can expect to manage approximately four after-hours calls per hour from nursing home staff, registered nurses, and directors of nursing.

Key Skills:
Strong typing skills are crucial as you will document interactions in real-time for each case.

If you hold an active Ohio or Wisconsin license, you’ll enjoy the liberty to work remotely while tackling challenging cases.

Primary Responsibilities:

• Use a provided telephonic platform to coordinate care, making both incoming and outgoing calls between facilities, hospitals, and Optum field colleagues.
• Utilize EMR systems effectively to provide acute care to members during specified hours.
• Manage both medical and behavioral health conditions alongside primary care physicians or specialty providers and ensure appropriate documentation for each encounter.
• Monitor and adapt care plans in response to changing member needs and family situations.
• Engage in effective communication with internal and external stakeholders, including physicians, members, and nursing facility staff.
• Meet scheduling requirements and attendance expectations consistently.

Required Qualifications:

• A valid, unencumbered NP license for Ohio or Wisconsin, free of any unresolved disciplinary actions.
• Active national certification from ANCC or AANP in Family, Geriatric, Adult, or related certifications, or the ability to secure national certification and NP licensure by your start date.
• Current DEA licensure or the ability to obtain it post-hire as per state regulations.
• Willingness to fulfill assigned weekend and holiday shifts.

Preferred Qualifications:

• A minimum of two years of postgraduate experience in Long-Term Care.
• Familiarity with geriatrics and chronic illness management.
• Understanding advanced illness care and end-of-life discussions.
• Proficient in using computer systems for documenting medical information.

The annual salary for this role ranges from $104,500 to $156,000, determined by various factors including local market trends and individual qualifications.
