Join our dedicated interdisciplinary team as a Telecare RN, an essential player in providing compassionate support to patients and families outside of regular business hours. In this dynamic role, you will assess and address the physical, psychological, social, and spiritual needs of those seeking assistance, ensuring comprehensive care through the nursing process, the VITAS Palliative Care Guide, and established Telecare Protocols.
You’ll be instrumental in delivering clinical assessments and interventions, acting as an advocate for patients and families. Your expertise will facilitate communication with attending physicians, long-term care facility staff, case managers, and other external colleagues, ensuring a seamless flow of information and care. Collaboration with LPN/LVN colleagues will be essential, as you’ll coordinate service delivery after hours, providing patient and family education, orchestrating interdisciplinary visits, and liaising with external vendors and resources.
Moreover, you will play a vital role in supporting Telecare staff, enhancing patient outcomes, and delivering bereavement support to families navigating difficult times. Documenting all interactions thoroughly in patient records is a key responsibility, including assessments, care plans, agreements, actions taken, and resolutions. Reviewing entries from Telecare LPN/LVN’s and Patient Care Coordinators to ensure documentation standards are met will also be a part of your role.
As you orient new team members and actively participate in in-services, staff meetings, and educational opportunities, you will foster your professional growth, setting and reviewing goals with your supervising manager monthly. Upon meeting qualifications, you will pursue certification in hospice nursing to further enhance your expertise.
You will ensure the proper use of medications, medical equipment, and supplies, optimizing resources from contract personnel, including Pharmacy and transportation services. Your commitment to customer service will reinforce an excellent care delivery experience, while adhering to all departmental standards, policies, and procedures relating to workload management, disaster planning, and quality standards.
To thrive in this role, you should hold at least two years of experience in acute-care nursing in areas like medical-surgical, oncology, home health, or emergency services. While one year of customer service or call center experience is preferred, strong verbal, written, and interpersonal communication skills are essential, as is the ability to manage conflicts and crises effectively. A solid foundation in professional nursing principles, relevant state and federal regulations, and the ability to work weekends and holidays is required.
You must have graduated from an accredited Nursing program, holding a current Registered Nurse License free of restrictions. If you’re located in a Nurse Licensure Compact State, obtaining multistate license designation is necessary. Be prepared for potential additional licensure based on business needs.
