    Join our dedicated team in providing compassionate care through locum hospice and palliative care telemedicine services in Oregon. This remote position offers a flexible schedule designed to suit your lifestyle, allowing you to make a meaningful impact from anywhere.

    Key Details:

    • Position Type: Remote Telemedicine
    • Location: Oregon
    • Start Date: ASAP – Ongoing, with full-time hours available; we welcome candidates who can commit to 2-4 weeks per month.
    • Schedule: Monday to Sunday, 8 AM to 5 PM, including on-call responsibilities.

    On-Call Requirements:

    • Weeknight on-call: Monday to Friday, 5 PM to 8 AM (typically averaging 0-5 calls/night).
    • Weekend on-call: Friday 5 PM to Monday 8 AM.

    Scope of Work:
    This role focuses solely on adult hospice and palliative care, involving the clinical oversight of 130-150 patients through our dedicated nursing staff who visit patients in their homes or facilities. Your responsibilities will include collaborating with hospital liaisons and the intake team to evaluate patient charts for appropriate hospice eligibility, issuing orders for medications upon admission, and completing the Certificate of Terminal Illness (CTI) shortly after a patient is admitted.

    Daily Responsibilities:

    • Review hospice admission RN notes and medical records through EPIC.
    • Conduct medication reviews for hospice-covered and non-covered drugs based on our preferred medication list.
    • Manage 2-6 admissions daily, with reduced volumes on weekends.
    • Engage in phone calls or telemedicine visits with patients for goals of care discussions or symptom management, usually conducted live with an RN present.
    • Assess the appropriateness for General Inpatient (GIP) care in consultation with inpatient teams and coordinate efforts with hospice staff and hospitalists.

    Meetings and Coordination:
    Participate in Interdisciplinary Team (IDT) meetings scheduled every Tuesday morning and biweekly on Thursdays. Duties also include signing prescriptions and faxes for facility orders. Familiarity with initiating and managing opioid infusions, ketamine, and methadone is desired.

    Technology and Support:
    You will utilize EPIC for electronic medical records and be backed by a full support staff, including RNs and hospital liaisons. A laptop can be provided to facilitate your remote work environment.

    Credentialing Process:
    Expect a straightforward credentialing timeline of 60-90 days, with no mandatory interviews unless specifically requested by the contractor.

    Qualifications:

    • Required: Board Certified in Hospice and Palliative Medicine (open to candidates from Family Medicine, Internal Medicine, or Emergency Medicine backgrounds).
    • Clean NPDB record without malpractice issues.
    • Ability to hold an Oregon medical license.
