    Location: Remote Salary: £46,000 to £53,000 (dependent on experience) Working Hours: 37.5 hours per week (part-time & compressed hours available)

    The Role

    In your capacity as a Clinical Pharmacist, you will be a vital member of our remote clinical team, committed to providing exceptional medication management, optimizing pharmaceutical care, and ensuring proactive patient support. Your influence will extend to implementing innovative initiatives that enhance our GP practices.

    Key Responsibilities

    • Provide top-notch, patient-centered pharmaceutical care that enhances health outcomes for all registered patients.
    • Collaborate with the multidisciplinary team (MDT) to devise innovative strategies that improve patient care and clinical efficiency.
    • Address prescription and medication inquiries swiftly and safely.
    • Conduct systematic medication reviews to enhance patient treatment regimens.
    • Oversee and manage repeat prescriptions, ensuring adherence to safe prescribing practices.
    • Facilitate medicines reconciliation for patients transitioning between various care settings.
    • Support chronic disease management by refining treatment plans for long-term conditions.
    • Lead and engage in quality improvement projects aimed at elevating patient care and achieving clinical excellence.
    • Promote a collaborative, ambitious, and innovative environment to cultivate the finest primary care services in the UK.
    • Maintain effective communication with patients, caregivers, and team members, ensuring professionalism and clarity in all interactions.
    • Thrive in a supportive clinical environment, with mentorship and professional development opportunities provided by senior clinicians and GPs.

    Who We’re Looking For

    We are on the lookout for a driven, innovative, and well-organized pharmacist who excels in a fast-paced, patient-centered environment.

    Essential Requirements:

    • Registered Pharmacist with valid registration from the General Pharmaceutical Council (GPhC).
    • Independent Prescriber qualification.
    • Experience in chronic disease management and prescribing.
    • Dedication to continuous professional development, demonstrated through ongoing training and education.
    • Post-registration experience in the primary care sector.

    Desirable Qualifications & Experience:

    • MSc in Advanced Clinical Practice.
    • Completion of the Primary Care Pharmacy Education Pathway.
    • Experience managing and prescribing minor acute conditions.
