    As a Medication Navigator, you will be at the forefront of supporting patients in their medication adherence journeys. This role is essential to improving patient experiences and health outcomes by ensuring that individuals can successfully fill, refill, and take their prescribed medications. You will work to identify challenges that inhibit medication adherence and guide patients through overcoming these obstacles. Your contributions will be pivotal to the Quality Team in achieving superior performance in key quality and patient experience initiatives.

    Work Schedule: 8:30 am to 5:00 pm EST

    Your Key Responsibilities:

    • Proactively connect with patients, healthcare providers, and pharmacies to facilitate the timely filling and refilling of critical prescriptions, particularly for managing cholesterol, hypertension, and diabetes.

    • Educate and empower patients to gain a clear understanding of their medication regimens and the benefits associated with them.

    • Serve as the go-to expert on pharmacy-related metrics, including MAD, MAC, MAH, SUPD, and SPC.

    • Resolve obstacles and streamline processes to ensure patients can fill or refill their medications efficiently, while connecting them with resources to cut costs, access mail delivery, and secure 90-day prescriptions. Act as a resource for non-clinical team members.

    • Achieve daily outreach goals through various communication methods (telephone, mail) with patients, providers, and pharmacies.

    • Review and audit medication adherence reports, addressing discrepancies with payors.

    • Engage in strategy meetings with Tri-State Leadership and payors to devise innovative solutions for enhancing medication adherence.

    • Contribute to the development of new initiatives and workflow advancements, particularly in managing high-risk patient populations.

    • Support quality measure initiatives, including chart reviews and data submission to payors.

    • Mentor and train non-clinical staff members to enhance team capabilities.

    • Perform additional duties as assigned to support overall team objectives.

    Qualifications We’re Looking For:

    • High School Diploma or GED (or higher).

    • A minimum of 1 year of patient-facing experience, ideally in medication reconciliation or management settings.

    • Proficient in Microsoft Office Suite, particularly Word and Outlook.

    • Access to a quiet home workspace that ensures the protection of Protected Health Information (PHI).

    • Residency in an area where you can secure high-speed internet access approved by UnitedHealth Group.

    Preferred Credentials:

    • Certification or license as a Pharmacy Technician, Medical Assistant, or Nurse, or possession of an Associate’s Degree (or higher) in pharmacy or nursing.

    • Bilingual proficiency in English and Spanish is a plus.

    • Experience with medical terminology (6 months or more preferred).

    The salary range for this position is $16.88 to $33.22 per hour for full-time employment, influenced by various factors like local labor markets, education, work experience, and relevant certifications.
