Chronic Care Manager (LPN) – Fully Remote Opportunity

This fully remote position is essential in delivering compassionate and proactive care to patients managing chronic conditions. Job Details:

• Employment Type: Full-time, 40 hours/week
• Schedule: Monday to Friday, with no evening or weekend shifts
• Compensation: Starting at $22.00/hour (fixed rate)
• Work Setting: Completely remote (please refer to tech requirements below)

Required Qualifications:

• Current LPN license (mandatory)
• Access to a personal Windows-based computer with webcam (tablets/iPads are not compatible)
• Stable home Wi-Fi connection
• Preferred experience in a remote work environment
• Exceptional organizational and interpersonal skills

Core Responsibilities:

• Perform thorough patient assessments to create tailored care plans
• Work collaboratively with interdisciplinary teams, comprising hospitals and primary care providers
• Track patient health status and arrange necessary follow-up care
• Provide education to patients and their families concerning self-management strategies, medications, and available resources
• Ensure accurate and timely documentation to meet all regulatory standards
• Act as an advocate for patient needs and coordinate services to guarantee seamless care continuity
