Position: Advanced Practice Provider (Nurse Practitioner / Physician Assistant)
CareNational is on the lookout for dedicated and knowledgeable Nurse Practitioners and Physician Assistants who are ready to enrich the lives of patients. This position is an essential element of our collaborative Care Team, working closely with Registered Nurses and Social Workers to deliver comprehensive care both in-home and virtually for individuals dealing with chronic kidney disease (CKD) and end-stage renal disease (ESRD/ESKD).
Your Impact
As an Advanced Practice Provider (APP), you will be instrumental in providing patient-centered care that emphasizes quality of life, proactive intervention, and effective disease management. Your key responsibilities will include:
- Performing thorough wellness assessments in patients’ homes and via virtual platforms.
- Guiding patients and their families through available healthcare programs, treatment choices, and preventive measures.
- Collaborating with primary care physicians, specialists, and additional healthcare services to ensure cohesive patient care.
- Managing treatment plans by prescribing medications and ordering diagnostic tests.
- Identifying and addressing gaps in care by liaising with physician practices to facilitate timely screenings and testing.
- Engaging in integrated care team meetings to formulate customized care plans for patients.
- Ensuring accurate and timely documentation in alignment with regulatory standards and organizational guidelines.

Roles & Responsibilities
- Provide evidence-based, patient-focused care that aims to minimize avoidable hospital admissions and enhance patient outcomes.
- Execute comprehensive health evaluations and craft personalized treatment strategies that cater to individual patient needs.
- Implement preventive care initiatives designed to decelerate the advancement of chronic illnesses.
- Track patient progress and ensure that follow-up care is appropriately arranged.
- Offer necessary education and resources that empower patients to navigate their healthcare experience.
- Uphold high-quality care standards in accordance with best practices and compliance regulations.
Requirements
- Possession of an active and unrestricted RN and NP/PA license applicable to the state of practice.
- Must hold board certification (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate is required.
- At least 2 years of direct patient care experience, ideally in managed care, chronic disease management, or primary care settings.
- Preferred experience in Managed Care/IPA/Health Plans.
- Bilingual skills (English/Spanish) are highly desirable.
- Must have reliable transportation, along with a valid driver’s license and car insurance.
- Familiarity with electronic medical records (EMR), data-driven patient management, and telehealth technologies is essential.
- Excellent communication abilities to effectively engage with patients, families, and healthcare teams.
- Skill in working independently while contributing to an interdisciplinary care team is crucial.
