    Job Description

    Position Overview:

    At Eventus WholeHealth, we are committed to redefining healthcare for adults living in diverse environments such as skilled nursing facilities, assisted living centers, independent living arrangements, and their homes. Our innovative model of integrated care is delivered by a dedicated team of healthcare professionals, including Physicians, Nurse Practitioners, Physician Assistants, Care Team Advocates, and in-house Support Staff. Leveraging telemedicine technology, we connect with patients using videoconferencing tools to facilitate real-time medical assessments, consultations, and treatment plans tailored to individual needs. Our telemedicine providers ensure that medical services are administered in accordance with specific site guidelines while maintaining ongoing communication with families, facility staff, and specialist providers when necessary.

    Key Responsibilities:

    • Deliver comprehensive primary care and acute psychiatry services following Eventus site-specific protocols, adhering to the highest standards of family, geriatric, and internal medicine. Perform additional tasks as assigned by Eventus Management to guarantee quality and timely medical care and administrative support. Build and maintain relationships with facilities and partners, manage patient consents, care coordination, and billing operations.
    • Exhibit clinical expertise and critical thinking to effectively assess and respond to the needs of residents in Eventus facilities. Prioritize multiple incoming messages based on urgency to ensure appropriate and timely responses.
    • Facilitate triage services in line with established protocols. Ensure that consent for treatment and telehealth services are obtained prior to virtual consultations. Adhere to established telehealth etiquette practices, involving family members when appropriate. Be proficient in the use of telemedicine equipment and documentation systems, promptly completing all necessary records post-visit and managing narcotic prescriptions as per Eventus RX Policy. Notify the patient’s primary provider about the visit and follow up with the treatment plan as directed by the triage schedule.
    • Foster open and effective communication with facility staff and administration. Provide clear verbal and written instructions regarding medications and care plans. Allow time for questions from patients, families, and staff, and establish a follow-up plan for patient care.
    • Adhere rigorously to Eventus policies and procedures, along with local, state, and federal regulations governing healthcare practices. Commitment to ethical standards and compliance with regulatory bodies is essential.
    • Maintain professionalism at all times, refraining from gossip, negative remarks, or inappropriate relationships, while presenting oneself in appropriate attire during calls.
    • Take on additional tasks and responsibilities as necessary, assigned by Eventus Management.
    • Uphold patient confidentiality with the utmost integrity, ensuring proper use of communication tools in compliance with HIPAA regulations.
    • Maintain active practice licenses and adhere to each state’s regulatory requirements as instructed by Eventus.
    • Focus entirely on the responsibilities of the role during scheduled shifts, as this position does not require on-call duties.

    Knowledge Requirements:

    • Thorough understanding of principles, practices, and regulations relevant to quality patient care.
    • Familiarity with the rules and regulations established by governing health care organizations.
    • Awareness of Eventus WholeHealth policies and operational procedures.
    • Knowledge of safety protocols to establish and maintain a secure working environment.

    Essential Skills:

    • Ability to gather and analyze patient data across multiple dimensions—physiological, socioeconomic, and emotional.
    • Skillful in accurately assessing patient needs and providing suitable guidance, interventions, or referrals.
    • Proficiency in documenting clinical interactions and services.
    • Competence with computer technology and software used in a medical setting.
    • Capacity for exercising independent judgment and discretion in clinical settings.

    Educational Qualifications:

    • Master’s degree in Nursing from an accredited institution, alongside an active advanced practice nursing license in the state of practice. Valid certifications related to your role are mandatory. Experience in geriatric, family, or internal medicine is preferred. Candidates must be licensed in VA or OH upon hire, with additional state licenses required within four weeks.

    OR

    • Completed an accredited physician assistant education program, passed the national certification exam from the National Commission on Certification of Physician Assistants.
    • Must hold a valid license in VA or OH upon hire, with additional state licenses required within four weeks. Preference for specialization in geriatric, family, or internal medicine as well as experience in ALF or SNF settings.
